The estimate will be based on the health insurance plan and the procedure(s) you select. The estimate will clearly show the estimated price of the procedure(s), your health coverage benefits, including deductible, co-insurance and co-pay, plus your estimated out-of-pocket responsibility amount.

If you don’t have insurance or are not using insurance, you have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment and hospital fees.
You must request a Good Faith Estimate from your provider at least one business day before your medical appointment or procedure. You can request the estimate before you schedule the visit or procedure.
Make sure to save a copy or a picture of your Good Faith Estimate. If you receive a bill that is at least $400 more than your Good Fait Estimate, you can dispute the bill.
