Billing and patient care records for 9-1-1 EMS incidents must be requested from the provider that created and maintains the record.

The EMS Agency is not the Custodian of Records for 9-1-1 patient care or billing records.

Request these records directly from the provider that created and maintains them.

All San Mateo County EMS service areas, excluding the City of South San Francisco and San Francisco International Airport (SFO) American Medical Response (AMR) Ambulance patient care, transport and billing records
City of South San Francisco South San Francisco Fire Department First-response and ambulance transport records
San Francisco International Airport (SFO) San Francisco Fire Department – Airport Division for all calls; American Medical Response (AMR) for transported patients Request first-response records from San Francisco Fire. If the patient was transported by EMS, request patient care and billing records from AMR.

 

Request Records from AMR

AMR uses MediCopy to process patient and authorized third-party requests for patient care and billing records.

  1. Open the AMR MediCopy request page.
  2. Review the authorization, identification and supporting-document requirements for your request.
  3. Complete and submit the request directly according to the instructions provided by MediCopy.
