San Mateo County Health partners with community members through boards, commissions and initiatives that guide programs, shape policies and promote equity
Public Health, Policy & Planning
Since 2018 San Mateo County Health has partnered with the Health Plan of San Mateo (HPSM) and Brilliant Corners (a housing-services nonprofit) to place medically fragile individuals experiencing homelessness into subsidized housing.
County Health’s Whole Person Care pilot originally developed this initiative with HPSM to enhance housing stability for medically fragile unhoused individuals with Board allocated Measure K funds of $2 million. At present, this program is managed by the Complex Care Programs under Public Health, Policy & Planning.
The subsidy is available to San Mateo County Health clients in need of housing support to enhanced stability and ensure access to necessary health care services. Once approved for the housing subsidy, clients are referred to Brilliant Corners through Community Supports program, which assists in finding suitable residences and administers direct rental subsidies. This system, paired with case management services from a variety of County Health and community based organizations staff, helps residents maintain their housing and supports improved health outcome.
