Accessing MyChart

If you already have a MyChart account, you can click here to log in.

A MyChart activation code will be printed on a recent After Visit Summary or on your new patient enrollment letter. Go to the MyChart website.

Enter MyChart Activation Code
Click on “sign up” and manually enter your MyChart activation code.

Enter Personal Information
Enter your zip code and date of birth, then click on “next.”

Set Up Two-Step Verification
You will be prompted to set up two-step verification, which is required.

If we sent you a text message or email with your MyChart activation code, click on the secure link. It will open a browser window on your computer or mobile phone.

Create your MyChart Username
Choose a username that is unique, but easy for you to remember.

Create your MyChart Password
Your password must include at least:

• One lowercase letter
• One uppercase letter
• One number
• One special character

Verify Information
Enter your date of birth and click submit. Review and agree to the terms and conditions.
