Forms for mandated reporters

SOC 341, as adopted by the California Department of Social Services (CDSS), is required under Welfare and Institutions Code (WIC) Sections 15630 and 15658(a)(1). This form documents the information given by the reporting party on the suspected incident of abuse or neglect of an elder or dependent adult.

Adult Protective Services: Home Safe Program

The Home Safe Program was established by Assembly Bill 1811 to support the safety & housing stability of seniors and adults with disabilities who are current Adult Protective Services clients or in the intake process and who are experiencing or at risk of homelessness. San Mateo County’s Adult Protective Services will administer the Home Safe program funds until June 30, 2025.

Program Requirements—Eligible clients must meet all criteria:

  1. Is an APS client, or in the APS intake process
  2. Is experiencing homelessness or is at imminent risk of homelessness; and
  3. Voluntarily agrees to participate in the program
