EMS Certification and Accreditation

Hours/Dates of Office Operation


EMT-I and EMT-P applications are accepted only on Tuesdays and Thursdays between the hours of 8 to 10 am and 2 to 4 pm. No appointment is necessary. If you are not able to come between the hours listed above, call (650) 573-2774.

225 37th Ave, San Mateo, CA 94403.

Directions to Vital Statistics

Please enter the San Mateo County Health System by it’s main entrance on 37th Ave. Pass through the automatic large double wooden doors. Vital Statistics is located on your left as you enter the building. Tell them that you are here for certification/accreditation and ask that the EMS on-call be paged.

Checklist: What You Will Need to Bring

EMT-1 Certification (Initial Certification in San Mateo County)

  • Completed EMT Application (Application)
  • Copy of EMT Course Completion Certificate
  • CPR/AED Card/American Heart Association/American Red Cross for the Professional Rescuer
  • Completed Request for Live Scan Form (Link Inserted Here)
  • National Registry Card and proof of successful exam completion date
  • Driver’s License, California ID, or U.S. Passport

EMT-1 Recertification

  • Completed and signed EMT Application (Application)
  • Copy of current EMT Certification Card
  • CPR/AED Card/American Heart Association/American Red Cross for the Professional Rescuer
  • Verification of EMT Skills Form
  • Verification of 24 hours of Continuing Education from a California Approved CE Provider (at least 12 hours must be instructor based)
  • Driver’s License, California ID, or U.S. Passport

Out of State EMT-1 Applicants

Paramedic Accreditation

  • Please contact your employer for the application process
  • Accreditation is only granted to paramedics employed by a San Mateo County ALS Provider
