Health Care for Homeless/Farmworker Health

The San Mateo County Health Care for the Homeless & Farm Worker Health (HCH/FH) Program Co-Applicant Board meets monthly, typically on the second Thursday of the month from 10:00 a.m. - 12:00 p.m. Please check the board packet for details and meeting locations. 

About HCH/FH
The Healthcare for the Homeless Program in San Mateo County was started in 1991 to provide health care to women experiencing homelessness and struggling with substance abusing in East Palo Alto. The Program has grown to coordinate medical, dental, and behavioral health care services for people experiencing homelessness throughout the County. In 2010, the Program’s responsibilities broadened to include farmworkers and their families. The renamed Healthcare for the Homeless/Farmworker Health (HCH/FH) Program is a Public Health Act Section 330(g) (h) program and receives federal funds from Health Resources and Services Administration (HRSA) to support and promote health care for these target populations. The HCH/FH Program also confers “Federally Qualified Health Center” status upon San Mateo County Health’s outpatient clinics.

Governance
The HCH/FH Co-Applicant Board serves as the governance structure for the Health Care for the Homeless/Farmworker Health Program in accordance to its bylaws and in conjunction with the Board of Supervisors and SMMC Board of Directors per the Co-Applicant Agreement . The Co-Applicant Board makes decisions about Program operation, development, implementation, and evaluation. It meets monthly. Board Members must reside in San Mateo County and cannot be employed by San Mateo County Health.

Program Contracts & MOUs

HCH/FH program has several contracts and MOUs with the County departments and community-based organizations to provide primary care, behavioral health, enabling, and dental services for people experiencing homelessness and farmworkers and their dependents. Below is a description of each contractor’s and MOU’s services and schedule.
