Program Contracts & MOUs

HCH/FH program has several contracts and MOUs with the County departments and community-based organizations to provide primary care, behavioral health, enabling, and dental services for people experiencing homelessness and farmworkers and their dependents. Below is a description of each contractor’s and MOU’s services and schedule.

Public Health Policy & Planning (PHPP)

1. SAN MATEO COUNTY STREET MEDICINE
To reach homeless individuals where they are, the Street Medicine program offers comprehensive services to people experiencing homelessness, whether they are living on the streets or in tent encampments.

Schedule: Monday, Tuesday, and Thursday at various locations in San Mateo County

Services:

  • Primary medical care
  • Complete physical exams
  • Laboratory studies
  • Medication management
  • Vaccinations
  • Wound care
  • Syringe exchange services
  • Portable nebulizer treatment
  • Ear lavage
  • Alcohol and substance use treatment
  • Psychiatry services
  • Bagged lunches provided by Samaritan House
  • Provision of cold weather supplies

 

2. SAN MATEO COUNTY FIELD MEDICINE
Aims to provide comprehensive healthcare services to farmworkers and their families at the places where they live, eat, and work.

Schedule: Pescadero: Monday-Thursday, Half Moon Bay: every Thursday

Services:

  • Primary medical care
  • Complete physical exams
  • Laboratory studies
  • Medication management
  • Vaccinations
  • Wound care
  • Syringe exchange services
  • Portable nebulizer treatment
  • Ear lavage
  • Alcohol and substance use treatment referrals
  • Comprehensive case management
  • Provision of cold weather supplies

 

3. MOBILE CLINIC
Serves San Mateo County residents who have Medi-Cal, Medicare, ACE coverage, or no insurance. A fee may be charged for specific services if no insurance method is available. The Mobile Clinic provides services to adults aged 18 and over.

  • Urgent care
  • Primary care
  • Screenings
  • Vaccinations
  • Physicals (please bring forms)
  • Family planning
  • Referrals for mental health and other health services
  • Syringe exchange services

 

4. ALCOHOL AND OTHER DRUG SERVICES (AOD)
Program offers drug and alcohol case management and counseling services in collaboration with the Street/Field Medicine team.

Schedule: Monday through Friday, 8:00 am to 4:30 pm

Services:

  • Drug and alcohol counseling
  • Screening, Brief Intervention, and Referral to
  • Treatment (SBIRT)
  • Referrals to Integrated Medication-Assisted
  • Treatment (IMAT), detox, and substance
  • abuse treatment programs
  • Screening appointments
  • Mental health referrals
  • Assistance with transportation
  • Care coordination with internal and external
  • service providers
  • Direct client care and seamless transition to
  • various AOD programs

 

ABODE SERVICES

Provides wellness specialists who support individuals who have recently transitioned from homelessness for up to 12 months. The specialists help connect them to healthcare services in San Mateo County, including medical, dental, and behavioral healthcare.

Schedule: Monday to Friday, 8:00 am to 4:00 pm

Services:

  • Medical care coordination
  • Assistance with health resources
  • Help with appointment scheduling
  • Vouchers for vision needs
  • Attendance at medical and dental appointments with client approval
  • Assistance with medical coverage transfers or medical care coverage
  • Lyft to and from medical appointments

 

ALAS

ALAS’s Farmworker Program is designed to improve the lives, health, and mental well-being of Coastside farmworkers and their families. We provide resources and support various wellness needs, including health promotion workshops on topics such as nutrition, blood pressure, stress, asthma, and diabetes. Our goal is to empower workers and their families to lead healthier lives.

Schedule: Monday to Friday 9:00am-12:00pm, 1:30pm-5:00pm

Services: 

  • Health Promotion Workshops offered directly at farms
  • Connection to ALAS mental health services
  • Referrals and linkage to external primary, dental, and behavioral services, including health insurance support, Coastside clinic, and Saturday dental clinic

 

Behavioral Health and Recovery Services (BHRS)

1. HCH (BEHAVIORAL HEALTH CARE COORDINATOR)
Assessing and linking homeless clients to medical, mental health, and/or substance use treatment. We act as liaisons between clients and their healthcare providers and provide peer support to homeless individuals.

Schedule: Monday-Friday

Services: 

  • Assessing and linking homeless clients to medical, mental health, and/or substance use treatment
  • Acting as a liaison between clients and their healthcare providers
  • Providing peer support to homeless clients

 

2. HEAL (HOMELESS ENGAGEMENT ASSESSMENT AND LINKAGE)
Provides field-based mental health therapy to homeless clients who are interested in the service but not ready to link to a clinic. HEAL offer groups and workshops to support individuals’ mental health needs.

Schedule: Monday-Friday

Services: 

  • Conducting mental health assessments
  • Offering clinic support to seriously mentally ill individuals
  • Referring individuals to the Health Plan of San Mateo for behavioral health services
  • Providing field interventions to engage clients in clinic services
  • Supporting clients in applying for housing and attending housing interviews
  • Conducting outreach, engagement, assessment, and linkage for mental health and substance use treatment

 

3. EL CENTRO DE LIBERTAD
A mental health and substance use disorder treatment provider, aimed at helping farmworkers and their families, youth, unhoused individuals, and the community of Half Moon Bay since 1999. The launch of the Cariño Project establishes a firm commitment to the Latinx farm workers and the
Coastside Community as a whole.

Schedule: Monday-Thursday 9AM-7:30PM

Services:

  • Drug and Alcohol Counseling
  • Co-occurring substance use treatment.
  • Assessments of clients’ needs
  • Case management
  • Early intervention/Prevention Education
  • Individual counseling sessions and/or psychoeducational groups
  • Outreach and engagement to the farmworker community
  • Facilitate linkages and referrals.

 

LifeMoves

COMMUNITY HEALTHCARE OUTREACH WORKERS (CHOW)
LifeMoves HCH community healthcare outreach workers (CHOW) assist clients in connecting to healthcare services, including insurance, dental, and behavioral health. We strive to provide comprehensive care and support to individuals experiencing homelessness.

Schedule: Monday-Friday 8:00am-4:30pm

Services: 

  • Outreach in the community to locate and connect with referrals
  • Needs assessments
  • Referrals to the San Mateo County Street Medicine team
  • Assistance in connecting with primary care physicians and specialists
  • Referrals for vision and dental care
  • Connection to health insurance
  • Referrals to behavioral health services

 

Puente de la Costa Sur

Puente is dedicated to providing support with free health coverage enrollment assistance, access to health services, and critical support. With a trauma-informed care approach, we strive to reduce health disparities in rural, coastal towns including Pescadero, La Honda, San Gregorio, and Loma Mar.

Schedule: Monday-Friday 8:00am-5:00pm

Sonrisas

Sonrisas Dental Health, Inc. is a non-profit dental center committed to providing access to quality dental care and oral health education to our community. Sonrisas deliver these services with dignity, respect, and compassion. Sonrisas’s vision is to ensure that every adult and child in San Mateo County has a dental home, regardless of physical, developmental, or economic challenges.

Schedule: Pescadero or La Honda on Wednesdays, 8:00am to 5:00pm

Services: Sonrisas’ staff offers a wide range of preventative and restorative dental care in Spanish at the Pescadero or La Honda clinic.

 

Saturday Dental Clinic

Saturday Dental Clinic provides comprehensive dental services for farmworker adults and farmworker families. Limited comprehensive care slots are available for homeless individuals.

Schedule: First Saturday of every month

Services: Range of comprehensive dental care services provided at no cost to the client.
