CalAIM Enhanced Care Management & Partners for Independence Program

General information

The CalAIM Enhanced Care Management (ECM) and Partners for Independence (PFI) programs provide social and health care management for people ages 21 or older who are eligible for Medi-Cal and are Health Plan of San Mateo (HPSM) members.

Both CalAIM ECM and PFI programs are voluntary programs providing intensive care management to older adults and/or people with disabilities with medical and psychosocial needs. 

CalAIM ECM and PFI goals include:

  • Preserve the clients self-determination to remain home safely for as long as possible,
  • Promote positive client outcomes through intensive case management,
  • Provide coordinated care using a collaborative approach with service providers and
  • Preventing or delaying out-of-home care.
